We’ve moved to a new HUB!

We would like to inform all our patients that we are moving HUBS from 1st July 2020.

 Appointments at Poplars Surgery must be booked in advance through your own GP practice (Reservoir Road Surgery) and are available with a range of clinicians including GP's, nurses and health care assistants. Please ask a receptionist for further information.


Following NHS guidelines all of our GP appointments are being converted to telephone consultations as of 17/03/2020

If the GP feels that they need to have a face to face consultation with you they will book an appointment to do so.

Prescriptions can be requested online or via email: reservoir.roadsurgery@nhs.net

If you have not yet nominated a pharmacy for your prescriptions to be sent to, you may want to consider doing this now. We will continue to provide updates to our patients via our website.

Out of Hours  
Always telephone - 465 2950.  The out-of-hours service is for emergencies only, when the surgery is closed

Home Visits
Requests for home visits should be made before 10am where possible, by telephoning 465 2950. The receptionists will ask you for the reason for the visit; this is to allow the doctor to assess the urgency.

Test Results
If you have had a blood test, urine check etc, you will be asked to either telephone or come to the surgery for the result. Unless the doctor has told you to call before, please try us after 1.00 pm on 465 2948.

 Extended Access 

COMMENCING 6TH JULY 2019 THE SURGERY WILL BE OPEN ON A SATURDAY FROM 8.30 AM-12 PM FOR BOTH ROUTINE AND EMERGENCY APPOINTMENTS

 Blood tests

NHS England have therefore issued guidance to GPs that all non-urgent blood tests should be halted.

Anyone who needs a test for urgent health problems will still get one, but where your clinician recommends that it’s safe to do so, then you may be asked to come back for a test at a later date, or your appointment may be rescheduled. 

Given the nature of the shortage, we cannot give an exact date for when the test will be rescheduled, but please be assured that if your condition or symptoms require it, then you will get a test, and we will be re-booking your test when supplies become more easily available.

 If your condition or symptoms change or get worse, please contact the NHS as you would normally.

Order Your Repeat Prescription Online

Please note, with effect from the 1st April 2022, we will no longer be taking requests for prescriptions via Email. 

How to order your repeat prescription from the 1st April 2022; 

Whichever method you use for ordering a repeat prescription, it takes 48 hours to process. 

How to order your repeat prescription 

Online 

The easiest way to order a repeat prescription is online https://www.reservoirroadsurgery.co.uk 

Simply fill out the Repeat Prescription Request Form 

In person 

You can do this by returning the right hand tear-off slip from a previous prescription with the required medications ticked, or by submitting a handwritten request. 

Please put this repeat prescription request in box outside the surgery, or hand it to the Receptionist.. 

By post 

You can post your prescription slip or written request to us at the practice. You can include a stamped addressed envelope for return by post if you will not be able to pick up your prescription from the surgery. 

Please note we are unable to accept repeat prescription requests by telephone. 

Electronic prescribing 

Instead of issuing a green hand signed paper prescription, the doctor can now send the prescription electronically to your chosen pharmacist. 

This means you don’t have to come back to the surgery to pick up the prescription, but you can go straight to the pharmacist and pick up your medicine. 

This works even better if you request the medicine online as it means you can miss out on two trips to the practice. 

You can nominate your pharmacy: 

  • Visiting your regular pharmacy and signing up for electronic prescribing. 
  • Ask the receptionist at the practice to register your chosen pharmacy. 

 

Medication reviews 

The doctors at Reservoir Road Surgery review the medication you are taking. They will sometimes call you in for a medication review and this may involve blood tests. It is very important that you attend these appointments, as it keeps you safe whilst taking medication.

We are upgrading to the next phase of the Electronic Prescription Service (EPS) which will mean that almost all our prescriptions will be processed electronically from 21/4/2020

To order your repeat prescriptions, please present your computer slip or written request - including your name, address and medication required - to the receptionist. Alternatively you can post your request to us, including a stamped address envelope for return or follow the link to the secure online form below.

Please allow several days for posting either way. Your prescription will be ready for collection 48 hours later.

Please order your prescription in plenty of time to allow continuity of treatment. We find it very difficult to process prescriptions “immediately” because medication has run out. If you do run out of your regular medication please ask your usual pharmacist, as they may be prepared to give you a small supply ‘on loan’ until your prescription is ready.

Normally antibiotics or oral contraceptive pills are not repeated without being seen. The doctors will wish to review your condition and medication periodically and you will be asked to make an appointment before the next prescription becomes due, please make this appointment in good time.

Electronic Prescriptions Service




A new system has been created to make it easier for you to pick up your repeat prescription. It is called the Electronic Prescription Service, or EPS for short. It is now available at Reservoir Road Surgery.




The Electronic Prescription Service is an NHS service that sends your prescription from your GP surgery to a Pharmacy without the need for a paper copy.




Nominate a pharmacy of your choice and this pharmacy will receive your prescription directly from your GP, via the Electronic Prescription Service (EPS).




With EPS you will not have to visit your practice to pick up your paper prescription anymore.




You will have more choice about where to get your medicines from because you can choose a pharmacy near to where you live, work or shop. Please only nominate a pharmacy if you are sure that you will always collect your medication from that pharmacy.




EPS is reliable, secure and confidential.




Your electronic prescription will only be seen by the same people in GP practices and pharmacies who see your paper prescription now.




If you are interested in using the Electronic Prescription Service speak to your pharmacist or GP practice staff.

3RD PARTY CONFIDENTIALITY & CONSENT

All staff have an obligation to safeguard the confidentiality of personal information. This is governed by law, contracts of employment and in many cases, professional codes of conduct. A statement of the duty of confidentiality is signed by all staff who have access to personal information whilst at the practice.

If you wish to allow a third party i.e family member etc to collect your prescription please complete the third party confidentiality and consent form - this will be scanned into your record and noted by our staff. You can withdraw consent at any time by informing the surgery. 

Please ensure you state fully who can be involved and to what extent.
