Their duties include visiting patients of any age in their homes, giving practical assistance or offering professional advice. Each patient's need is assessed and the care/treatment is planned accordingly.

Health Visitors

The health visiting team give advice and support to families with young children, expectant mothers, the elderly and patients with a disability.

Acute Clinical Team

Our ACT nurse supports and reviews patients with complex long-term medical conditions who are at risk of episodes of frequent ill health. This care is delivered in the patient’s home or in the community.
