If you would like access to this new feature, you will need to download and fill in the application form available in the link and bring it to reception with two forms of ID.  The requirements:

Two forms of ID - one must be photo ID and one must have your current address - please note that a driving licence is considered as only one form of ID.

You will be able to have access to a summary of your medical history, your prescription history and any recorded allergies.  However, to be able to do this you will need to update your access authority by visiting the surgery to present proof of identity before the appropriate access codes can be attached to your records.
