New Patient Registration
The receptionists will be happy to guide you through the procedure. When you do register, to speed up this process, we ask that:
- You live within our practice area (see map below)
- Please collect registrations forms from the practice. There are two forms that require to be completed. These are issued and taken in between the hours of 11am and 4pm. As these need to be checked please do not post these to us, or leave them with the receptionist before being checked, as this will cause a delay with your registration.
Should you be restricted with the collection time or unsuccessful in obtaining a registration form, please write to Wendy Hempseed Practice Manager. A decision will be made following a meeting with the GP Partners and you will be contacted in due course.
