Online 'Registration Form' With The Practice

If you live within the Practice boundary area (which covers roughly an 8 mile radius – a more detailed map is available in the surgery) and wish to register with Ravenswood Surgery click on the link below to open the form. When you have completed all of the details, please email your form/s to ravenswood.tayside@nhs.scot.

Once we have checked if we can accept your registration, you will be sent further forms to complete your registration with the Surgery.  These must be sent back to us at your earliest convenience to enable us to deal with your application promptly.
