New Patient Registration

If you live within our practice area and would like to register at Oakfields Health Group (Oakfields Health Centre & Dipton Surgery) please ask for a registration form and information pack at Reception.

You will be asked to fill in a form and a simple health questionnaire so that the doctors and nurses have some information about you immediately.

You will be offered an appointment with the nurse or our health care assistant for a new patient “health check” shortly after registering with the practice. Please note that failure to attend the new patient health check could invalidate your registration.

Health checks provide an assessment of your height, weight, blood pressure and discussion/advice about lifestyle such as diet, smoking, alcohol consumption and levels of physical activity.

If you are on repeat prescriptions for on-going medicines you may be asked to make an appointment with the doctor. We suggest that you try to obtain a month’s supply of your medicines from your previous practice to give us time to process your new repeat prescriptions.

Oakfields - Allensford, Blackhall Mill, Blackhill, Bridgehill, Burnopfield (outskirts), Chopwell, Coalburns, Ebchester, Hamsterley colliery, Hat & Feather, Hedley on the Hill, High Spen, Kiln Pit Hill, Low Westwood, Medomsley, Medomsley Road, Rowlands Gill (outskirts), Shotley Bridge, Victoria Garesfield

Dipton- Dipton, Flint Hill, Tanfield Lea, Tantobie, Stanley, South Stanley, East Stanley, South Moor, Burnopfield, Annfield Plain, Catchgate, 

The shaded area on the map represents Oakfields Health Centre practice boundaries this will be updated in the near future to include Dipton Surgery boundaries. We will accept patients within these boundaries. Patients who are already registered with us but move outside the practice boundaries will be asked to register with a practice closer to home. Please take note of the ID that is required to complete your application and bring this to the surgery with your completed forms to register.
