New Patient Registration

When registering, you will be asked to complete a registration form for each person registering with the Practice. In addition, all patients will be asked to complete a new patient questionnaire, allowing us to provide medical care in the interim period, while your medical records are transferred from your old practice to this one. 

Please note that the new flats on Wellheads Avenue and Wellheads Road are outwith our area, so unfortunately we are unable to accept applications from these addresses.

If you move out of the practice area, you will need to register with a new practice as soon as you move. 

Please download the forms you need, fill them in (preferably typed rather than by hand) and email them back to us. When handing back the forms please provide a copy of photographic ID (e.g. passport) and a copy of a recent letter with your name and address (e.g. utility bill, bank statement or rent agreement, etc). For children we require a copy of their birth certificate or passport. You can choose to download the form in PDF or Word format.

For each adult and child over 5 years old that you wish to register please download and complete the 'Adult New Patient Form'.  The General Data Protection form (the last page) has to be completed by any child aged 13 years or over and signed by them.

For each child under 5 please download and complete the 'Child Under 5 New Patient Form'.  

Once completed you can return the forms and proof of ID via email to gram.newdyceclinical@nhs.scot for the attention of Arlene Davidson.

It will take around 1 week to become registered, if you wish to call to confirm you have been registered please telephone us, and if there are any issues we would telephone or email yourself. We will not call you to confirm that you have been registered successfully.

Practice Boundary

Please use the boundary map to check if your house is within our area for accepting new patients. If you think that your house may be within our boundary, but aren't sure after looking at the map then please telephone reception on 0345 337 9922 to enquire further.

We will:

  • Ensure our patients have 24-hour access to medical advice.
  • Aim for you to have access to a suitably qualified professional within 48 hours of your initial contact, or in an urgent case, the same day.
  • Work in partnership with you to achieve the best medical care
  • Involve you and listen to your opinions and views in all aspects of your medical care.
  • The prevention of disease, illness and injury is a primary concern. The staff will advise you of steps to promote good health.
