New Patient Registration

Register Currently Capped

The Partners at Pound Hill Medical Group have taken the decision to cap their patient list in order to manage the demand of registered patients.

To register with a local GP surgery, please contact the NHS Allocations team on 07425 616194 or email sxicb.gpallocations@nhs.net

Once the NHS Allocations team receive your allocation form, they will let you know which GP practice you have been assigned to. This may be Pound Hill Medical Group, or it may be another surgery in the area. You will then need to complete the registration process of your allocated surgery. You will need to provide your allocation letter with your registration forms.

If the NHS Allocations team allocate you to Pound Hill Medical Group, you can then register with the practice as described below. When we have received your forms and an email or letter confirming the allocation, you will be registered.

Further information can be found on the Sussex ICS website.

How to Register

To register as a Patient with this Practice you need to be living within the Pound Hill, Worth and Copthorne areas as shown on the map held at each surgery. Please ask at reception if you are unsure whether you fall within our catchment area. You can see our catchment area below and check your postcode.

You must:

  • Either provide a medical card with details of your new address in the space provided and a contact number or complete the purple registration form (GMS1). You must fill out the whole form. If you are unsure, we have examples at reception to help.

Also requested:

  • A utility/credit card bill stating your local address and some form of photo ID to allow us to sign you up to the online services.  In case of a family, one person's ID will suffice unless the surname of the adults are different.
  • A completed New Patient Questionnaire.

If you need to see a doctor before you can provide this information, you will be directed to the local GP walk-in Service at Crawley Hospital who will be able to help.

If you are on any repeat medication you must make an appointment to see a GP.
