Registration

When you register as a patient, we will ask you to show us/send us some form of identification (ideally a photo), and proof of where you live. For this reason, the surgery will not accept postal applications for registration.

Both a GMS1 and Questionnaire must be completed for each individual applicant.

When you register with the Practice, you will be registered with a specific Doctor and this will be recorded in your medical records. Please ask one of our reception team if you wish to know the name of this; your accountable GP. This does not mean that you will see this GP every appointment. If you have a preference for a particular GP, please let us know (in writing) and we will try to accommodate your request.
