New Patient Registration

Please collect a registration form and new patient pack from our reception during opening hours.  Please bring a form of ID with you when returning the completed forms.

Out of Area Registration

Out of Area registration has been stopped, and we are currently contacting all of those patients who are out of area to inform them that they need to obtain a new GP within their local catchment area.  

Some time ago a small addition was made to our catchment area that included the Chase Farm Estate, and this has now been removed.  We will honour the registrations that were made during that time, but unfortunately will no longer be accepting registrations from this area.
