We are happy to register patients who live within our practice area. We serve the villages of:-

We are using a new online service that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number at this point.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Registering by paper form is still available please complete the documentation and return to the surgery along with two forms of identification, a photographic form of ID (such as a passport or driving license) and proof of your home address (such as a recent bank statement or utility bill).
Also we we can offer a Dispensing Service for your medication to patients living in Elmesthorpe, Sharnford, Thurslaston & parts of Sapcote. Please ask at reception for details.

You will be allocated a named accountable GP, this will be allocated to you once you have been registered with the practice in writing. This is purely an administrative procedure. We have an open policy here for patients, you may make an appointment to see whichever GP you wish.
In addition, if you are aged 75 or over you will also be notified, in writing, of a named GP who will have overall responsibility for the care and support that our surgery provides to you.
