Register online

1) Download and complete the registrations documents below

We need your full term time address, including room and block number.
You will need to upload a digital picture of your signature to these forms. You can do this by saving a picture of your signature and then adding the picture to the form.
If you are having problems completing the forms, you can collect a registration pack from reception.

2) Email the completed forms to contact.w92025@wales.nhs.uk.

If you take regular medication, please include your repeat prescription form with your application to register. For more information on prescriptions please see here
