Registering With The Practice


New Patients


The Practice is currently welcoming new patients. The Practice operates between Tramways Medical Centre and Middlewood Medical Centre and patients are registered with and will be expected to attend services at both Practices. Patients are eligible to register if they reside within our Practice boundary (the red line). An outline of this can be found below:

Once your registration has been completed you should receive a message from the Practice confirming this. If we experience any difficulties with your registration or require further information from you, you may be contacted by a member of our reception team.

If you do not have access to the internet, you can register by visiting either Practice and completing one of our registration packs. The pack contains a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.  Once you have completed your forms, please return these to the Practice is person.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

We are prooud to be a "Safe Surgery" for everyone in our community. All are welcome. Everyone in the UK has the right to free care from a GP. If you don't have proof of ID or address, you can still register. Immigration status or nationality don't matter - we won't as for immigration documentation and won't share your information with the home office unless serious crime is involved. In partnership with Doctors of the World UK, we aim to ensure that our Practice offers a welcoming space for everyone who seeks to use our services.

If in the future you move outside the red boundary or the blue extensions then you may be asked to re-register with another GP closer to your new address.

Changing Your Address


Any current patient who changes their address is encouraged to update this with the Practice as soon as possible, to ensure that any important communications are able to reach you. A change of address form is available from either of our Reception desks. Existing patients are eligible to remain with the Practice so long as your new address resides within either the red or blue outer boundaries.
