You will need to complete and return the following:
Pre-registration form (GMS1)- Registration form
- Online Medical Questionnaire For New Patients: This questionnaire provides useful information whilst we wait for your medical records to arrive from your previous doctor. [Sometimes it can take a considerable time for us to receive your medical records. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.]
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
You will also need to provide a copy of:
- EU Nationals - we require a valid passport and proof of address.
- Asylum Seekers - we require Home Office document, HC2, a reporting schedule, ID card and proof of address.
Whilst we normally require original documents for proof of identity, in view of the coronavirus pandemic; we are accepting copies of these by email or post.
Medical treatment is available from the date of registration. Please contact reception for further information.
Returning the documents
Please scan or alternatively take a clear and legible picture of the completed registration documents together with your proof of identity, and send them to us by email to or post.
