Use online Patient Triage

To see a health care professional GP at the practice you need to make an appointment.  We can usually offer an appointment on the day and you can expect to see a doctor within 48 hours.  If you wish you can book up to 20 weeks ahead. You can expect to see a nurse within 24 hours. 

To make a routine appointment at surgery please call 0121 4110362 between 09:00 and 13:00.  You can arrange an appointment for our morning surgery between 09:00 and 13:00 at Summerfield Primary Care Centre,

or 17:00 and 18:00 at Summerfield Primary Care Centre.

If your appointment is urgent please tell the receptionist who will offer you an urgent appointment slot if all routine appointments have been filled.

If more than one member of you family needs to be seen please let us know so we can arrange a longer appointment.

If you are using a pay phone please tell us and we will call you back.

08:00 – 18:30 after consultation hours:

Please ring surgery and you will be connected to the duty clinicians.  

Telephone Consultations

You may not always need to see the doctor in person.  If you need advice you can book a telephone consultation leaving a number you wish to use and the doctor will call you at an agreed time.  If you are only available on a mobile phone you will be expected to call the surgery at the agreed time.

Cancellations

If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.

Home Visits

If possible please try to telephone reception before 10:00 if you require a home visit.

You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed.

You can be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.

Specialist and Hospital Care

If a member of our team believes you need hospital treatment or specialist care elsewhere, they will ask you where and when you would like to go.  They can then book your appointment electronically while you wait for most services. 

If you would prefer to have some time to think before deciding where and when to have treatment, you will be offered the option of calling the practice later and we will be able to book your appointment then.

Test Results

If you have any investigations done such as blood tests you should telephone the practice within a few days for the results.  If for any reason, we require you to come and see us you will be notified of this when you are given the results. 

Test results will only be given to the patient, their parent or guardian, unless they give prior consent for the result to be given to a named person.
