Practice profile:

We are a large multi-disciplinary team based over 4 sites with a registered list of just over 16,500 patients.

The practice area covers four ex-mining villages and the large Hardwick Hall estate, bordered to the East by the M1 motorway and to the West by the Peak District National Park. We have first class facilities in a purpose built premises at Tibshelf and refurbished and extended sites at Stonebroom, Pilsley and Holmewood.

We are a friendly integrated team that has been committed to training for over 20 years. We have 6 GP partners and currently employ 5 salaried GPs. The Partners have special interests in Dermatology, Women’s Health, Diabetes, Cardiovascular Medicine and ENT amongst others.

The Primary Healthcare Team is further made up of 5 nurse clinicians, a community matron, a care co-ordinator, practice nurses, treatment room nurses and Healthcare assistants. The Practice is supported by our experienced team of administrative and reception staff.

We are fortunate to co-locate with our health visitors and district nurses and we have a range of additional services that take place at all of our sites e.g. CAB, physio’s, psychotherapy services, podiatry etc. We provide full minor op, minor injury, joint injection and cryotherapy services.

We have a multi-professional learning ethos and hold weekly educational primary health care team and clinical supervision meetings, adopting an inclusive approach to education. We employ a dedicated Training Co-ordinator whose role it is to co-ordinate the placements

of our GP Registrars, F1 & F2 Doctors and other medical students to ensure our placements provide the best possible leaning opportunities. Feedback from our previous trainees has been extremely positive.

The practice has a history of innovation and being at the forefront of trailing new ways of working. We have our own Community Support Team that supports our most complex and frail patients with the aim of avoiding admissions and providing the highest quality integrated multidisciplinary care.

The practice is a very pro-active member of the CCG and two of our Partners undertake Committee and Governing body roles with the CCG. Our Practice is a high achiever in the Quality and Outcomes Framework and National GP Patient Survey.

The Practice offers extended hours to patients, as well as participating in a range of clinical enhanced services.
