Practice Profile:
The practice operates from purpose built (1994) premises in the centre of Matlock with 4 GP Partners and 2 GP Assistants. These house not only the practice staff but our entire district nursing & health visiting, all of whom have direct access to our EMIS Web computer system through which we try to work as paperless as possible. Close by is a strong and accessible Community Mental Health Team. Monthly MDT meetings are held including representatives from local social services.

Monthly Palliative Care meetings are also held with representation from palliative care nurses at the local Hospice.

The practice has teaching status for medical students, GP Registrars, nursing and health visiting students.

Appointments for doctors are booked up to one week in advance. A Duty Doctor is booked each day for urgent cases. Appointments for nurses can be booked in advance. We offer late and early appointments on Monday and Tuesdays which can be booked online via patient access. Prescriptions can also be ordered via patient access.

We care for approximately 8000 patients and 300 temporary residents per annum and operate all usual clinics one would expect to find. Increasingly, the routine intermediate reviews of patients with chronic conditions are being protocolised and handled by practice nurses. Protocols are available on the practice intranet.

One mile away lies Whitworth Hospital to which the practice has a significant commitment. There are 18 well used GP beds on Oker Ward. The doctors perform weekly ward rounds to oversee care of patients on the ward.

The hospital has a minor injuries unit seeing approximately 7,500 new attendances per annum. Most of the minor A&E work has now been protocolised and is handled by Nurse Practitioners

Although Whitworth hospital contains a maternity unit, none of the GPs are involved in intra-partum care. Since September 1999 the unit has been midwife led and we have no obstetric commitment to it.

Our General practice out of hours work is conducted through Derbyshire Healthcare United which covers the whole of Derbyshire including the Peak District and all of the community hospitals.

We have been a training practice since 2009. GP Registrars have their own consulting room and work closely supported and in addition to GPs.

Dr Emmerson is also a programme director for the Chesterfield Training Programme.

Avenue House and Hasland Partnership is based in central Chesterfield and serves 20,500 patients. We have 7 GP partners (2 of whom are trainers) and currently employ 5 salaried GPs.

The partnership has 2 sites. GP Registrars can be based at Avenue House Surgery and/or Hasland Medical Centre. We also offer placements for third year medical students from Sheffield Medical School.

The Practice strives to be progressive and innovative, whilst maintaining the core ethos of continuity of care and family care. We like to offer a supportive environment for learning and pride ourselves on a ‘practice wide’ approach to training within a positive and friendly atmosphere. Our registrars tell us that they feel well supported with easy access to their trainers. We hold daily ‘debrief’ sessions and weekly tutorials, tailored to individual needs.

The Practice has an excellent Primary Health Care Team including Nurse Practitioners, Practice Nurses, Health Care Assistants and an in house Phlebotomist. Working very closely with the practice staff, are the community staff, including District Nurses, Health Visitors and Midwife. The Practice is supported by our experienced team of administrative and reception staff. The team meets as regularly as possible and has a proactive approach to improving patient care.

In addition to a range of GP appointments being offered (pre-bookable and book on the day), we also offer Nurse Practitioner appointments plus a range of chronic disease management clinics, run largely by the nursing team. We have access to welfare rights advice, citizens’ advice and counselling services.

Our Practice is a high achiever in the Quality and Outcomes Framework and also achieves well under patient access/appointment targets and in patient questionnaires.

The Practice offers extended hours to patients, as well as participating in a range of clinical enhanced services. We take an active role in Practice Based Commissioning and are members of the Chesterfield Consortium of local practices.
