Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to complete a travel questionnaire which can be collected from our reception desk or completed online:

Once you have completed the form and submitted to the practice, the practice nurse will go through your form. If you require any vaccinations an appointment will be booked with you.

It is important you complete your travel questionnaire as early as possible - at least 8 weeks before you travel. These vaccines have to be ordered as they are not a stock vaccine.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.
