In an effort to encourage supported self management, the team at Barns Medical Practice have produced service specifications for some of the more common chronic diseases. The aim of these documents is to highlight the condition and give details on the service that you can expect to receive from this surgery if you have a diagnosis of that condition. They provide some useful information and signpost you to recommended websites for further reading. It is anticipated that these will be updated as new evidence or guidelines become available and each specification has a review date highlighted in an effort to keep them current. As this is a new development we would welcome feedback from anybody accessing these web pages.
