If you have received an invitation to attend the surgery for annual health check, please read this information before you attend. For an easy read guide to the annual health check please click here

Please follow this link to a checklist with questions that would be helpful for the doctor or nurse to know before your appointment. Please complete the checklist and email it to evergreen.oak@nhs.net 

Alternatively, you may drop a printed copy to your usual surgery before your appointment.  If you cannot email a copy or drop a printed copy to surgery, please print a copy and bring it with you.

Please bring a urine sample, in any clean container, with you when you attend for your health check.  This is to check for Diabetes.

It is very important that you have your flu vaccination when you are invited. If you have not already had your flu vaccination, you may be able to have it during your health check.  The doctor or nurse will advise you on when best to have the flu vaccination.
