ACCESSIBLE INFORMATION

The Accessible information Standard aims to ensure that people who have a disability have access to information that they can understand and any communication support that they might need. 

At the Lathom Road Medical Centre we want to make sure that we are communicating with you in a way that is easy for you and that you can understand. Please let us know if you:

  • Need information in a specific format e.g. Large print or easy read?
  • Do you need to receive information in a particular way,i.e. text or email?
  • Do you need someone to support you at appointments e.g. sign language, interpreter or a carer or friend?
  • Do you lip read or use hearing aid? 

We will do our best to ensure that your needs are met and with your permission record this in your file and share the information with other NHS and adult social care providers.
