If you require any vaccinations relating to foreign travel you require to fill out and hand a completed travel risk assessment (attached below) at least 6 weeks prior to travel.  Once we receive your completed form you will be contacted by a member of our travel team who will provide information on the recommended immunisations with timescales and be able to answer any queries you may have.  You will then be required to make an appointment in order to receive the injections for which there may be a charge as detailed below in the travel information and charges attachment.

If you are travelling within 6 weeks you need to contact one of the private travel clinics which are listed below in the attached document.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS.  See attached document below with details of charges.
