We have a new social prescriber working with us who can now help with many social issues. Some of which include:

  • Mental Health: Anxiety, Depression, Emotional drain, Mood Swings
  • Community Transport: Application – Blue Badges/Disable Parking, Bus Passes and Renewal, Hospital Appointments Transport Link, Free Bus Service
  • Children & Family Wellbeing Support: For those who care for children with medical challenges, parents and carers of Disabled/Physically Challenged Children. Counselling Support Linking Carers and Mothers with support giver
  • Housing Association/Council Housing Support Letters: eviction, Homeless, Moulds, Damps and Disrepair Accommodation
  • Equipment: Equipment Funding Charities, Wheelchair, Steps Adaptions, Wheelchair Access to Homes, Home Adaption, Grab Rails, Bed Rails, Toilet Aids, Bathing/Showering Aids
  • Meals and Shopping Services: Food Banks and Food Vouchers
  • Improve Mobility – Physical Activities/Exercise Support: Dance Class, Swimming, Cycling, Walking
  • Carers Support: Respite Care
  • Social Isolation: Befriending, Local Social Clubs, Dance Classes, Connecting with the Community
  • Employment Support: Developing Employability Skills, Adult Learning
  • Home Safety/Security: Alarms, Alert/Emergency Buttons
