We require at least 8 weeks notice to complete holiday vaccination forms.  Once your form is submitted, we aim to complete the form with at least 4 weeks remaining before the proposed holiday date to give you time to book in to have any appropriate vaccinations.  Please note though, we complete forms based on the proposed holiday date not the date you submit the form.  For example, if you submit a form with 12 weeks notice and we receive a form a week later with 8 weeks notice, we will complete the latter form first.

Any patients submitting forms with less than 8 weeks notice, will be advised to contact a travel clinic for advice as we may not be able to complete the forms in time to provide adequate vaccination cover for the proposed holiday date.  If you choose to leave the form with the Practice, please note we cannot guarantee this will be completed in time for your proposed holiday.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  These could include: Yellow fever, Japanese encephalitis, malaria etc.  This is because not all travel vaccinations are included in the services provided by the NHS.
