If you have been advised by the surgery to submit an annual review of your asthma symptoms please use this form. If your symptoms are deteriorating or you are having any concerns please make an appointment with our Nurse.
Asthma Review
Usually women aged 50 to 70 are invited for breast screening every 3 years. Currently, women aged 47 to 49 are also being randomly invited as part of a pilot project. Our patients will be screened at North Middlesex Hospital or Forest Primary Care Centre.

Be Breast Aware
Breast Screening information
All men and women aged 60-74 are invited to carry out a faecal occult blood (FOB) test. Every two years, they're sent a home test kit, which is used to collect a stool sample. If you're 75 or over, you can ask for this test by calling the freephone helpline on 0800 707 60 60.

Children and Adult Immunisations are carried out by the Practice Nurses. Additional Adult Immunisations can be given by the Health Care Assistants e.g: Shingles/ Flu.
Please note: When attending Child Immunisation appointments, please remember to bring their red book so that we can update the child’s records.

This clinic is run by Dr M Northrop on a Tuesday afternoon or Wednesday morning and by the practice nurse Patrica every Thursday.
- Diabetic checks & Diabetic foot assessments
This clinic is run by the practice nurse Patrica every Thursday. To book yourself into the clinic please call the reception. Please note these appointments are not open to the general public and are only used for diabetic reviews.
you will need to book an initial consultation with the Dr in order to assess what type of ear drop you will need. Once a have attended this appointment a second consultation will be given with either the Drs' or the Nurse in the afternoon.
Any forms will need to be handed to any member of the reception team. It will be passed onto the doctor where they will assess if you need to come in for an appointment. There is a charge for this service. Please see Non-NHS Services under the Patient information Tab.
Seasonal flu is a highly infectious viral illness that can cause serious complications in vulnerable people/people suffering from long term chronic conditions.
A vaccine against various strains of flu is available every year and the Department of Health recommends immunisations for ‘at risk’ groups.
If you are: over 50 years of age, Living in a long stay residential home, A carer, Pregnant or Severely overweight with a BMI over 40
If you suffer from a long term chronic condition such as: Chronic (long term) respiratory disease (severe asthma requiring regular use of steroids, COPD or bronchitis), Chronic heart disease, Chronic kidney diesase, Chronic liver disease, Chronic neurological disease, Diabetes, Weakened immunity system due to disease or treatment such as steroid medication, cancer treatment, HIV/AIDS or No spleen or dysfunction of the spleen.
If your child is aged 2, 3 or 4*: This year the Department of Health (DOH) recommends immunisation of ALL children aged 2, 3 and 4 (including healthy children). This is in the form of a nasal spray.

The doctors out health checks for all new patients and Over 75’s.
If you are aged between 40-74 years old and do NOT suffer with a chronic disease (i.e. diabetes) then you are eligible to have a free NHS health check.
The nurse can provide a NHS health check for you in 15 minutes. These checks aim to help you lower your risk of developing common but often preventable disease(s).

Once you have given birth, you will need to register the birth within 6-8 weeks or as soon as possible. We will invite you for an appointment with our Baby Clinic Administrator.
Whopping Cough Vaccination: The department of Health advise that all pregnant women between 20-32 weeks have a whooping cough vaccination. When you reach 28 weeks please contact the surgery on 0208 800 9837 or via Patient Access to make an appointment with the nurse or Doctor.
The Department of Health recommends Shingles Vaccination for all patients aged 70-71 or 78-79; this is a one off vaccination offering lifetime protection.
If you are due or eligible for the Shingles Vaccine please call the Surgery on 0208 800 9837. During flu season you can have this vaccine during your flu clinic appointment.
This vaccine should not be given to patients who are immune suppressed, e.g. patients who are on chemotherapy or high doses of oral steroids.

Patients can view their test results using our online services (you must be registered to use this service).
The Reception team are not medically trained and cannot offer any advice. All results are assessed by the Doctor. The Receptionist will confirm results to patients or tell you if you need to make an appointment or speak to the Doctor or Nurse.
- Smear test results can take 7-14 working days.
- Blood and urine results can take 2 – 10 working days.
- X-ray results can take 2 weeks or more.
If your test results have not arrived within the allotted time, please inform us and we will chase up your results.
Please call the surgery on 0208 800 9837 in order to get a prescription for any dressings you may need and also for a double appointment. These appointments are normally during the afternoon on a Wednesday or Thursday.
