If you plan to travel abroad, you may require certain vaccinations.  As soon as you know you will be travelling, you will need to complete a Holiday Health Planner which you can collect from Reception or download from the link below. Please note the holiday planner must be submitted 8 weeks before travel. Once the form is received, our Nurses will determine whether you require any vaccinations.  We ask that you telephone the surgery 2-3 days after completing the form to find out your requirements.  We advise you to enquire about vaccinations well in advance, as some need to be administered weeks or even months before departure.

This form must be submitted a minimum of eight weeks before travel. The Nurse will assess your travel plans, medical history and medication, they may deem it to be complex. If this is the case, then you may have to visit a private travel clinic.

Please be aware we only offer NHS funded travel vaccinations, those that require payment are considered a private service and must be given at private travel clinic.
