Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to complete the Travel Questionnaire below and submit this a minimum of 8 weeks prior to travel. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to complete this form as early as possible - at least 8 weeks before you travel - as an appointment will be required with the practice nurse once the form has been reviewed. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work. 

Please be advised we are unlikely to be able to process your request if the form is not completed and submitted at least 8 weeks prior to your travel date. In the instance we are unable to fulfill your request you will be directed to a private clinic where you will incur a fee.

Please note that not all travel vaccinations are available on the NHS.
