Sickness certificates
If you are absent from work due to illness for a period of up to seven days, and your employer requires a certificate, you should complete a self-certifying form. (SC2 Form). These forms are no longer available at the practice but can be downloaded from the HM Revenue & customs website at www.hmrc.co.uk. If you are ill for more than seven days, you must see the doctor to obtain a medical certificate.
Child health / immunisations
This clinic is carried out by one of the doctors on a Tuesday morning. Your child may become irritable and slightly feverish in the 24 hours after the injection. If this happens, give your child infant Paracetamol to lower their temperature. If you have any queries regarding appointment times or immunisations please contact our Office Supervisor on the main line.
Cervical Smear Tests
Cancer of the cervix can be prevented and all female patients between the ages of 20 and 65 are advised to have regular smear tests. These are important because they can detect early signs of the disease, which would then be easily treated. We operate a call and recall system to remind you about the tests, which are carried out by the Practice Nurse, usually on a Wednesday afternoon.
Flu Vaccination
Flu vaccinations are recommended for all patients over 65 years and also those with a chronic disease. Invitation letters are sent to all patients entitled to a flu vaccination by the practice.
Postnatal examinations
These are carried out by the Practice Nurse usually twelve weeks after you give birth. Appointment letters will be posted out to remind you of when it’s due.
Fees
Certain services provided by the doctor are not covered in the NHS, and you will be asked to pay a fee, e.g. private medical certificates, HGV/taxi licenses,insurance claim forms, fitness to drive etc. A list of fees is on display at reception.
