You can be referred to the Link worker by the GP, Nurse or GP Admin Staff

Bereavement

Applying of benefits

Appealing DWP sanctions

Domestic abuse

Coming to terms with a new diagnosis

Terrie focuses on the priorities of those patients by listening to their non-health related concerns, understanding the issues that are at the top of their agenda and helping them access the most appropriate support.  Your first contact with the link worker will always be a telephone call to discuss your needs
