 DIRECT ACCESS XRAY SERVICE RESUMES TO FULL WALK-IN SERVICE

We are pleased to inform you that University Hospital of North Midlands imaging departments are returning the x-ray service back to full walk-in (as per pre-pandemic service). A referral is still required from you GP but you no longer have to wait for an appointment date and time.
Those patients that have been referred for an XRAY prior to the 3rd October will be sent a letter to inform them that they can walk-in.

Prospective Access to Medical Records 

The NHS wants to give people better ways to see their personal health information online. We know that people want to be able to access their health records. It can help you see test results faster. It also lets you read and review notes from your appointments in your own time.

We’re now letting you see all the information within your health record automatically. If you are over 16 and have an online account, such as through the NHS AppNHS website, or another online primary care service, you will now be able to see all future notes and health records from your doctor (GP). Some people can already access this feature, this won’t change for you.

This means that you will be able to see notes from your appointments, as well as test results and any letters that are saved on your records. This only applies to records from your doctor (GP), not from hospitals or other specialists. You will only be able to see information from the date you register for online access. For most people, access will be automatic, and you wont need to do anything. 

Your doctor (GP) may talk to you to discuss test results before you are able to see some of your information on the app. Your doctor (GP) may also talk to you before your full records access is given to make sure that having access is of benefit to you. There might be some sensitive information on your record, so you should talk to your doctor if you have any concerns.

These changes only apply to people with online accounts. If you do not want an online account, you can still access your health records by requesting this information through reception. The changes also only apply to personal information about you. If you are a carer and would like to see information about someone you care for, speak to reception staff.

The NHS App, website and other online services are all very secure, so no one is able to access your information except you. You’ll need to make sure you protect your login details. Don’t share your password with anyone as they will then have access to your personal information.

If you do not want to see your health record, or if you would like more information about these changes, please speak to your GP or reception staff.

Please note: The above will not come into effect until 1st November 2022

Royal Stoke Imaging Wait Times

Patients are currently waiting longer for Royal Stoke Imaging Department appointments, please can we ask that calls to chase waiting times are directed directly to the imagining team on 01782 679285 which is the Helpdesk, they will provide the current wait times 


Join Our Patient Participation Group

We are currently looking for new members to Join our patient participation group,  (also called PPG).   The PPG is a group of patients who meet with practice staff during the year to discuss how our patients view the practice’s performance and to discuss what plans we put in place to improve our performance. The group is also contacted during the year if specific views are sought on various topics from the group. It is not an onerous task but allows you to understand better how a GP practice operates and gives you an opportunity to have a say on how the practice performs.

Coffee Morning

Drop in coffee mornings are being help on the second Friday of every month at Waterfall Village hall from 10:30 until 12:00. Transport can be arranged if needed by contacting Judith on 07511502765.


We are pleased to announce that our new entrance is now complete and open to patients.

The build started mid June 2021 and was due to be completed in 8 weeks. Unfortunately this extended to 6 months due to Covid which affected the supply of building materials. Thank you for your patience during this time.

There are two entrances – one for Reception (appointments and queries) and one for Dispensary (for collection of prescriptions or queries). Our electronic doors have improved our Disabled Access - please press the pad and wait a few seconds for the doors to open automatically. They will close behind you – there is no need to push/pull the doors.

Just a reminder of our opening times :

Reception is open 8 until 1pm and 2pm until 6pm Monday, Tuesday, Wednesday and Friday and 8 until 1pm on a Thursday. We are still available by telephone between 1pm and 2pm.

Prescriptions can be collected from Dispensary between 11am and 1pm and 3pm and 5.30 pm.

If you are eligible for your booster it is vital you get it ASAP, but please do not call the practice regarding this. You can book online here or find a walk in clinic here

REGISTERING FOR YOUR TYPE 1 OPT-OUT

 If you don’t want your identifiable patient data to be shared for purposes except for your own care, you can opt-out by registering a Type 1 Opt-out.

This collection will start on 1 July 2021 so if you do not want your data to be shared with NHS Digital please register your Type 1 Opt-out.

The data held in your GP medical records is shared with other healthcare professionals for the purposes of your individual care. It is also shared with other organisations to support health and care planning and research.

If you do not want your personally identifiable patient data to be shared outside of your GP practice for purposes except your own care, you can register an opt-out with your GP practice. This is known as a Type 1 Opt-out.
To register a Type 1 Opt-out, for yourself or for a dependent (if you are the parent or legal guardian of the patient) (to Opt-out) you can download the form here and then return it to the surgery.

Information about "Extended Access"

Under the Extended Hours Primary Care Services programme, there is a requirement for patients to be able to access 1½ hours additional appointments after 6.30pm each week day and an effective weekend service based on the local needs of the area. All practices must be able to direct patients to the service and offer a choice of evening or weekend appointments on an equal footing to core hours appointments.

We are pleased to announce that appointments are now available between 4pm and 8pm weekdays and clinics on a Saturday and Sunday at the following Hubs. These will be run by a team of GP Federation Locum GPs who will have access to your medical records following your consent to share. Please ask our Receptionists about this service as these appointments can only be booked via this Practice.
