Process of Registration

**PLEASE NOTE**

Due to ongoing high demand on all the Bathgate practices, we are accepting new patients on a rotational basis with the other two practices in the health centre building. There will be a sign on the door of the practice that is accepting new patients for that day. If you need to register with a GP, please come to the health centre in person.

Due to this current rotational arrangement we are unable to accept registrations online, though we hope to reinstate this in the future.

If you are a relative at the same address of an already-registered patient, or you wish to register a new baby, please bring identification and proof of address and see reception for forms to complete.

A form must be filled in for each person in the family wishing to register. You will be given a medical questionnaire form to complete containing some brief medical questions. Proof of address must be produced at the time of registering.

If you are on repeat medication, you will be required to make an appointment with a GP so that your medication can be reviewed.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
