NEW PATIENT REGISTRATION
We would ask that if you need to register, please complete the new patient registration form and a new patient health questionnaire for each patient. There is a separate health questionnaire for adults and children. Once completed you can either post to us or attend and put in the white post box outside the practice.
We receive a large number of applications each time we register new patients. In order to try to manage this better and to ensure your registration is not delayed due to an incorrectly completed application form we have introduced a registration time period. You are welcome to collect forms anytime but we will only accept completed application forms from 9.00am - 5.30pm, Monday to Friday.
The checking process takes some time and it is difficult to do this with the care and attention we would like both early morning, as this is an extremely busy time, and late afternoon, as there are less resources.
If you live within our practice area you are welcome to register with us. Our reception staff will be happy to guide you through the procedure. You will need to provide at least two forms of ID. One - proof of address, e.g. utility bill, driving licence, mortgage/rental agreement. Two - photographic ID, e.g. passport, driving licence.
You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. You can download the registration form from this website but you will need to come in to complete the health questionnaire.
It is our usual practice to accept new patients every second week, on a rota basis with Ladywell Medical Centre (West), with whom we share our building and some services. Should you attend when it is not our week to take new patients you may be asked to register with the West wing. If you already have family members registered at Ladywell Medical Centre East please explain this when you come to reception.
New patients may have a health check to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
You may print off a registration form, fill it out and bring it in with you when you register. Please note, we do not routinely email patients so this section does not require to be filled in. You will then be asked to complete a medical health questionnaire. This provides us with some medical information whilst we wait on your previous notes.

CHANGES TO PERSONAL DETAILS
If you move outside the area, we would ask you to inform the practice and register with a doctor in your new locality. Please also notify the practice of any change of name, address or telephone number, so that we can update your records.
It is particularly important to let us know if you change your telephone number as the practice now sends text messages to patients, if you have consented to this. It is your responsibility to ensure we have the correct details.
