New Patient Registration

We accept new patients who live within our practice area or Woodley, Bredbury and Romiley.  Please use the pre-registration tab to complete your registration form electronically.

Pre-registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. Before you register with us it is always best to get a full months worth of any medication from your current GP as it can take a little time for all your details to come through to us.

If would be helpful to know if you are a Care Leaver or leaving the Armed Forces, there is additional support we can offer both these groups of patients.  Please email us on stoccg.alvanleyadmin@nhs.net to let us know.  We have a Care Leavers programme that will help you to learn more about your new community and the wellbeing services we can support you with.  For Veterans we can direct you to the Veterans health service and mark on your referrals you are a Veteran which should mean you are directed to the right services quicker.  For more information on the Veterans health service please go to https://www.penninecare.nhs.uk/military-veterans-service

When you register you will also be asked to fill out a medical questionnaire. This helps us to understand if there are any acute issues that we need to be aware of.
