NEW PATIENT REGISTRATION 

Our Practice List is open and Sunnyside welcomes all new patients.  To register with the Practice you will need to complete a registration form and a health questionnaire which are available in the New Patient Registration Pack - this can be collected from the Surgery.    In the New Patient pack you will find a medical questionnaire for completion because it can take a considerable time for us to receive your medical records from your previous practice .    A registration form and questionnaire  must be filled out for each family member including children.  

It can take up to 10 working days to register your details on the practice system.  When this has been completed you will receive a text from the surgery to say you are now registered with the practice.
