** Re-opened for New Patient Registrations **

We have re-opened our practice to register new patients who live in our catchment area.

Please come along to Lander Medical Practice or Threemilestone Branch Surgery to register in person.  We will give you the necessary forms for registration.  Please read the information tab regarding Online Services Registration.  It is not presently possible to register with a GP at this Practice online, however we are developing this service and it will become available in the near future.

The Practice encourages you to see a Doctor soon after registering so that we can familiarise ourselves with your medical history and treatment.  Medical records can take a considerable time to reach us and this examination gives us valuable information about you until they arrive.  Please ask the Receptionist for an appointment with the Doctor for a New Patient Check.

New Patient Registration

The Practice is pleased to accept anybody who lives within the Practice Catchment Area. Patients will have a named GP, but if you have a “preferred” GP, please can you advise the staff, who will enquire whether or not this GP is able to accept you.  If you have expressed a desire to "opt out" of the care data or Summary Care Record, please can you inform the staff when you register, so that the appropriate codes may be applied immediately.

If you live outside our Practice Area, the Practice will no longer be taking any new registration requests and the existing patients, under the new rules from 5 January 2015, registered from out-of-area may not request Home Visits from a GP, Midwive or District Nurse.  Please make sure that this is a suitable registration option for you.  If a visit was necessary you would need to ring NHS 111. 

Please note: we will not action any 'out of area' registrations that you submit.  If you live outside the catchment area below, we will not process your application.  Please register with a GP closer to your home.  Many thanks
