Registration with the practice
If you wish to register with the practice you need to contact us - in person or by telephone - to make sure that you live in our catchment area.
The registration process itself is very simple.
When you visit the surgery for the first time you will need to show proof of address (anything with your name and address on it - like a phone / utility bill, tenancy agreement etc). We also accept registrations from people with no fixed address.
You will then be given a registration form to complete. Alternatively you can print off the registration form below, fill it out and bring it in with you.
The form asks for your NHS number. If you don't know it, you can contact your previous doctor. We need your NHS number for the electronic transfer of your medical records. We can register you without your NHS number but having this number will speed up your registration.
We will also ask you to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. Again, you can print off the form below, fill it out and bring it with you.
Addional Registration Forms
Once you have completed the paperwork you will be given an appointment for a New Patient Health Check.
We routinely offer tests for HIV and Hepatitis to our patients as part of the New Patient Health Check.
