Joining the Practice

registrationIf you live in the practice area and you would like to become a patient of the practice, you will need to visit the reception in person to collect a form called a FP1 which you should complete and return to the reception, along with photographic ID for EACH person registering and one proof of address per household.  When registering a child you will need to provide proof of name such as the child's Red Book, Medical Card or new baby NHS number & discharge details. 

Once you have returned the form it will be given to one of the doctors who will sign it agreeing to accept you as a patient. If the doctor refuses to sign the form (The most likely reason for this would be that you do not live in the practice area.) then we will not be able to accept you as a patient of the practice but we will help you with any urgent medical attention you require.

Once you have registered with the practice Essex Contractor Services will be informed and your medical records will be requested from your previous GP. Please note that it may take several weeks for us to receive your old medical records.

Please note that you may see any doctor within the practice not just the doctor you are officially registered with.
