New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also need one form of photographic ID if you require online access.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

In line with NHS England guidance children must be registered with a parent or legal guardian at the same address.

If you live outside of Tadcaster your prescriptions may go to our Dispensary rather than a chemist, please ask at reception for details.

When you register with the practice we ask that you book a 20 minute appointment with the Health Technician for a New Patient Medical. This will allow us to get some basic information about you onto our system e.g. blood pressure, height, weight and ongoing medical conditions. Children do not need to book one of these medicals.

Practice Area

We accept patients from Tadcaster and the surrounding villages including Appleton Roebuck, Barkston Ash, Church Fenton and Ulleskelf. Please contact reception for full details or click on this link

Unfortunately we cannot accept new registrations from patients in Aberford, Acaster Selby, Bishopthorpe, Collingham, Copmanthorpe, Hutton Wandesley, Linton, Long Marston, Micklefield, Rufforth, Ryther, Sherburn in Elmet, South Milford, Tockwith or Wetherby. Patients currently on our list who move to Aberford, Sherburn In Elmet or Copmanthorpe may be able to remain registered with us (please check with the reception team) but other than these locations patients who leave the Tadcaster area will have to register with a more local GP.
