New Patient Registration

Please ask the receptionist for a registration pack. When registering you will be asked to complete a simple questionnaire as your previous notes can take a long time to catch up with you.

On registering with the practice you are registered with a named GP, which is done on a rotation basis with the doctors.   It does not mean that you have to see that particular doctor, you can see any doctor in the practice.   The doctors ask that if you have been seeing them for an ongoing problem that you continue to see them.  If you have a preference please let the reception staff know. However we encourage patients to see the same doctor if possible as this helps with continuity of patient care.

We do not discriminate on the grounds of: race; gender; social class; age; religion; sexual orientation or appearance; disability or medical condition.

The Practice Area 

Our practice area comprises Hitchin, Letchworth (excluding the Grange and Jackmans Estate), most Hertfordshire villages within a 6 mile radius of Hitchin, and the Bedfordshire villages of Shillington, Stondon, Henlow Camp and Henlow. 

Our Outer Boundary 

The outer boundary is for patients that are registered with the practice and move to one of the areas below but would like to remain registered with the practice.

Online 'Pre-Registration' With The Practice

Please print off a registration form and registration pack for new patients, fill these out and bring it to the surgery in person as soon as possible with proof of your ID and proof of your residential address.

NHS Zero Tolerance Policy


We adhere to the NHS Zero Tolerance policy for abusive, aggressive and violent behaviour. Patients who are verbally or physically aggressive will not be tolerated and will be removed from our list.

Moving/Change of Address/Mobile Numbers
If you move, or change your name or telephone number, including mobile number please inform us.


Parking
We have limited parking facilities for patients when using the surgery. Please do not park in any bays that are not marked for Patient/Staff Parking.

Surgery Closure
The surgery may be closed for staff training from 1pm – 2 pm on the first and third Monday of each month.

Mobile Phones
Mobile phone use is not permitted in the surgery. Please turn off your phone when you enter the building.

Training
All Health Care Professionals may from time to time have with them students who are undergoing training; you will be asked beforehand if you agree to their presence. If at any time you feel unhappy about this you are not obliged to see them.

Confidentiality
Any information received by the doctors or staff is treated with the utmost confidentiality. If you wish to discuss any matter with the receptionist in private please ask.


Consent
When attending the surgery prior to a procedure, you will be asked to give your consent to the procedure, this may be written or verbal, but will be saved into your records.

Consent for Children
Everyone aged 16 or over is presumed to be competent to give consent for themselves. For children under the age of 16 a child who has ‘sufficient understanding and intelligence to enable him/her to understand fully what is proposed’ (known as Gillick Competence (Fraser Guidelines) then he/she will be competent to give consent for him/herself. Young people aged 16 and 17, and legally ‘competent’ younger children, may therefore sign a Consent Form for themselves, but may like a parent to countersign as well.

For children under 16, except those above, someone with parental responsibility should give consent on the child’s behalf by signing the consent form or giving verbal consent. This will be recorded in the child’s notes.

Records/Data Protection
Computers and written records are used for record keeping, recalls, repeat prescriptions, data collection and audit purposes. The practice is registered under the Data Protection Act and aims to adhere to the Caldicott Guardian guidelines regarding release of patient identifiable information.

Rights and Responsibilities
As an NHS patient of the surgery you have a right of access to general medical services, and for these to be delivered to a good standard. Correspondingly you also have a responsibility to use the service appropriately, and to treat our staff with due courtesy and respect.

Patient Allocated Name Accountable GP
From 1.4.15 practices are required for all patients to be allocated a named GP. Our patients who are 75 and over will have been informed already. When we register a patient at the surgery all patients are allocated a named GP, this is done on a monthly rota basis. There is no condition for the patient to see that particularly GP - patients are entitled to see any GP in the practice. The GPs do ask if you are seeing them for a particular problem that you continue to see them for that. If you would like to know who your registered GP is please ask at reception.

When you register you will also be asked to fill out a registration pack and medical questionnaire.  Please click to print this off and return to the surgery with your registration form.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
