New Patient Registration
We welcome new patients who live within the practice boundary.
Before joining the practice all new patients will be asked to fill in a medical questionnaire, provide their NHS number, proof of address and bring a an up-to-date photo identification. New patients requiring repeat medication will need to see a doctor to have this added to their record.
Your registration will actually be with the surgery but on your registration form you will be able to state your preference for a particular doctor.
Patients moving outside the practice boundary will need to register at another surgery.
