New Patient Registration

Colinton Medical Practice can only accept patients living within our practice boundary.

Unfortunately Colinton Medical Practice is declared ‘full’ which means that it is currently only able to register those new patients who are moving into the area, newborn to a family already registered, or asked to leave another Practice because you are outside that Practice’s boundary. If you are unclear whether you meet the criteria to register please enquire at Reception. Alternatively, information on which practice serves your address can be found at https://www.nhsinform.scot/scotlands-service-directory/gp-practices   or phoning 0800 22 44 88.  Temporary Residents to the area can be treated by the surgery.

Documentation Required For Registration

If you are new to the area and wish to register with our practice please bring along the following documentation before 1700:

Proof of identification (photographic)  and

A General Practice Registration (GPR) form. Click here:  General Practice Registration Form 0121

and

and

Colinton Surgery Ethnicity Form. Click here  Ethnicity Form 0121

Further general information on registering with a medical practice is available from the NHS Inform website.   Click the following link to obtain this: NHS Inform..../registering-with-a-gp-practice

You will be asked to show photographic proof of Identification.  We no longer insist on proof of address, except where we have evidence that you have moved to a new address outside our practice area.   The Reception Team will advise on the specific requirements.

When registering you will also be asked to complete our New Patient Registration forms and a Questionnaire as these allows the doctor to familiarise him/herself with your medical history and healthcare needs.   THESE MUST BE BROUGHT PERSONALLY INTO RECEPTION BEFORE YOU CAN REGISTER AS YOU ALSO NEED TO BRING PROOF OF IDENTIFICATION AND ANSWER SOME RELEVANT QUESTIONS (please don’t send them in – there are questions the receptionist needs to ask and she needs to see the proof of  identification).

Appointments and Medication After Registering

  • Appointments: Unfortunately we cannot register you and immediately give you an appointment as the registration process takes at least 24 hours. Therefore please allow 24 hours between registering and asking for your first appointment.
  • Medications: If you are on any medications on joining us, you will need to make an appointment with a GP once registered before any medication can be prescribed by our GPs.
