New Patient Registration

There is one GP Practice  within Oakley Health Centre – Oakley Medical PracticeAny individual who lives within the Practice boundary area may apply to register with the Practice. Simply request a new patient registration form from reception or download from the link below. The practice does not discriminate on the grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.

The registration process requires you to complete a new patient registration form, medical questionnaire and if you wish, attend a New Patient Health Check.

Patients will be registered with the Practice and not with a specific doctor.  Medical treatment is available from the date of registration. New babies should be registered with the practice as soon as possible after leaving hospital, please bring along your new born child’s birth certificate with you.

Please ensure you have at least 4 weeks supply of any regular medication before registering as it takes a few weeks for us to receive your medical records and we may not be able to provide prescriptions until we have these.

Please note: Identification is required to be seen by Reception staff when registering with the surgery. This should be proof of address and also Photographic identification. Examples of accepted identification are Driving Licence, Utility Bill, Passport, Birth Certificate.

Changes to Personal Details

Please advise us as soon as possible if you change your name, address or contact details.
