New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility needs to be confirmed and this can be done by providing proof of address e.g. recent utility bill, photographic I.D. e.g. passport and any other evidence of your free entitlement to NHS care e.g Visa or EHIC. Please complete and return your registration forms between 10.00 a.m. and 3.00 p.m. We can be extremely busy first thing in the morning and also in the late afternoon and so it is best to avoid these time if at all possible.
All patients over the age of 16 require to attend with the above documents in person. You will need to complete a Registration Form (GPR) and Health Check form. At times we may experience a high volume of new registrations. At these times we may not be able to register you immediately but will do so as soon as we can. If you feel that you need to be seen urgently, please let the staff know.
Please note patients are registered with the practice and not an individual GP.
Medical treatment is available (depending on entitlement) from the date of registration. Please contact reception for further information.
