New Patient Registration

If you live within our practice area (see below)you are welcome to register with us and our reception staff will be happy to guide you through the procedure. 

You will need to complete a registration form (GMS1) either online via this link: https://gp-registration.nhs.uk/M83122 (Please contact us if you have any problems with this)

and by completing a Online Medical Questionnaire For New Patients

Or

By obtaining a printed  GMS1 form from our reception team along with a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.


All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration.

Practice Area

We accept new patients from the vicinity of Waterhouses and the surrounding area including Alton and the following villages: Bradnop, Onecote, Grindon, Butterton, Cauldon, Cotton, Whiston, Ipstones, Foxt, Froghall, Kingsley, Swinscoe, Stanton, Wootton, Ellastone and Denstone.

In order to keep your records up-to-date and to enable us to contact you should we need to, please inform the Practice immediately of any change of address or telephone number(s), including mobile and/or place of work if applicable.
