New Patient Registration
If you live within the practice catchment area (see map opposite) you will be entitled to register with the practice.
Please come to reception to complete a registration form and new patient questionnaire (or download from the links below). Please note, new patient questionnaire is not applicable to under 16s.
You will need to be able to provide evidence of your current address in the form of a recent utility bill.
You will be given a health questionnaire to complete. This is so we have some background information on you whilst we are waiting for your medical record from your previous GP. Patients aged 16 and over only need complete the questionnaire.
If you are taking regular prescribed medication, you will be asked to make an appointment with the Practice Pharmacist for a medication review.
Patients who move outside the area will be asked to register with a more local GP surgery.

Named GP
Each registered patient at Lindley Group Practice has a named GP who is responsible for that patients overall care at the practice. If you wish to know who is your named GP, please contact the practice. If you have a preference as to which GP that is, reasonable efforts will be made to accommodate your request. However, a named GP (whether it is one of your choice or not), will not affect your care at Lindley Group Practice in any way.
Changes to Personal Details
It is the responsibility of patients to keep us informed of any changes (name, contact number, or address) so we can update your record.
If you move outside the practice area, you are advised to register with a more local GP surgery as soon as possible so your records can be transferred.
