Friends and Family Test (FTT)
Would you recommend our practice to your friends and family?
Think your views are important?........... So do we. Your opinions are very valuable to us. We will use them to try to improve our services and meet your needs.
With effect from 1st December 2014, the opportunity to provide feedback via the FTT will be available to everyone at all times therefore questionnaires can be completed by anyone making any type of contact with the Practice, ie in person, on the telephone, after a home visit. We are looking for continuous feedback so please do complete the questionnaire again even if you've done one before.
All responses for each month will be collated into a report which will then be published here on our website. No responses will be attributable to you but any written comments you make will be included in its entirety although all attempts will be made to remove information that could identify you. There is a box to tick if you do not wish your comments to me made public.
