Consent Form | Use this pack to request a letter from a GP providing evidence of a medical condition / your health to your workplace, university etc. Download document 1, 2 and 3 - please complete the steps and bring your request to the surgery. We cannot proceed with your request if you do not complete the necessary steps! (We cannot provide you with proof of identity or address letters) This is for requesting a medical evidence letter only from a GP; not copies of records or medical history reports/forms. |
Request for a medical report / subject access request / medical history forms / copies of records | Individual Rights / Subject Access Request Form (must be completed) Individual Rights / Subject Access Request Patient Consent Form (you only need to complete this form if you wish to share your information with a third party, or if you are requesting records on behalf of another patient) | Use this to request access to your records, such as a copy of your medical records. This can also be used to request completion of a medical report (for insurance, army, DVLA etc.) If you are requesting access to a patients records (not your own records), you must submit a completed consent form too.
