The Patient Contract

We aim to provide a high standard of service to all our patients. In order for us to maximise the service we are able to give, we request patients agree to a number of practical measures.

  • To access appropriate medical care please use the online health query form whenever possible to keep our phone lines free for urgent care.
  • When completing the online health query, please provide as much information regarding your symptoms as possible. This will assist the GP in appropriately triaging the request.
  • Strictly one appointment for one patient only. 
  • To endeavour to attend appointments punctually and to cancel any appointments with plenty of notice when unable to attend - failure to do so on 2 or more occasions may result in your removal from our practice list.
  • To adhere to appointment timings and durations. The scheduled appointment is for the health query submitted another health query would require to be submitted for subsequent problems.
  • To note that home visits are only for housebound patients who are too ill or frail to attend the surgery (transport responsibility remains with the patient).
  • To treat all practice staff and doctors with courtesy and respect. We work as a 'ZERO TOLERANCE' practice.
  • Patients must always endeavour to do whatever possible to improve their own health.
  • To keep the practice informed of up-to-date addresses and telephone numbers, especially when moving house.
