The Great North Care Record is a way of sharing medical information across the North East and North Cumbria; this is accessed by Health and Social Care Practitioners.

Key information about your health is shared between different healthcare services such as hospitals, out of hours and ambulance services. 

Key information about your health include diagnoses, medications, details of hospital admissions and treatments.

Your Great North Care Record will be accessed if you are receiving treatment; the people treating you will ask your permission to access your Great North Care Record. Your record will only be accessed with your permission. You have the option to provide your permission at the point of care or, if you prefer, you can opt of the programme all together.
