Northampton Activity on Referral Scheme

A Quick Guide for Patients

Activity on Referral is a countrywide fitness scheme which is available to join through the surgery.  The scheme is based around a reduced rate leisure membership and combined 12 week programme of physical activity.

Who is Eligible?

Activity on Referral is available to people over 16 years of age who are currently inactive. Patients can be referred for a range of reasons including; being overweight/obese, issues with smoking, those suffering from depression, anxiety, mild bone and joint problems, patients with Parkinson, MS, diabetes, asthma, hypertension and back pain (see below for a full list for referral criteria). 

If you would like to be referred to this activity scheme please email woodview.k83040@nhs.net or call the surgery on 01604 670780 and speak to one of the receptionists who will arrange your referral.  You will not need to see a GP before the referral is made. If you wish to be referred Woodview Medical Centre will share some of your personal data with Trilogy Leisure so they know how best to support you.  Without your consent to share this information we are unable to refer you via this scheme.

The information provided will only be used as part of the Activity on Referral scheme to ensure that the scheme is delivered within the County standard protocol guidelines. All data will be stored securely.

What happens next…….

Trilogy staff will make contact with you within 5 days

You will be offered a consultation appointment within 7 days

You will be able to access a 12 week exercise programme and have access to five leisure centres during the 12 weeks:

    • Lings Forum Leisure Centre
    • Danes Camp Leisure Centre
    • Mounts Baths Leisure Centre
    • Duston Sports Leisure Centre
    • Cripps Recreation Centre ( Northampton General Hospital Grounds)

Patients will be able to use the fitness facilities at their convenience, with specifically designed exercise programmes to meet their exercise needs. Suitably qualified staff will prepare a bespoke programme of exercise for you after an initial discussion.

Staff will be on hand to support you at any time and will meet on a prearranged dates after 6 weeks to monitor your progress and at 12 weeks/programme completion.

On completion, you will be provided with advice on how to continue with regular physical activity.

COST

    • Consultation Fee - £20.00 (fee includes consultation meetings with an exercise professional at 1,6 & 12 weeks)
    • Scheme Cost - £17.50 per month, direct debit for 3 months committed membership

Please note this is a saving of £8.50 per month in comparison to one of the similar   membership packages (£26 per month committed for 12 months) !

Scheme Entitlement –Use of the Five Trilogy gyms access to Four swimming pools, over 200 fitness classes and two Health suites and the Forum Cinema

Referral Criteria 

Patients are only eligible for the referral if they are over 16 years of age, Live a sedentary lifestyle and meet at least one of the referral criteria outlined below:

Cardiac Disease Prevention

People with family history of heart disease but no current symptoms or other risk factors

High Cholesterol

Moderately raised readings >6.5mmol/l or no medication

Stress/Depression

Mild to moderate sufferers (if you have a support worker or carer they are also required to attend)

Diabetics Type I or II

Who are controlled through dietary restriction alone, or on drug therapy and condition is stable

Impaired strength or mobility/osteoarthritis/ osteoporosis

Providing the condition is not so far advanced that there is a risk of fracture, 

Obesity

BMI over 30 or over 28 with co-morbidities

Hypertensives

Controlled BP less than < 160/100mmhg

Cystic Fibrosis

Low / moderate symptoms.

Neurological conditions

Static neurological conditions such as Parkinson's Disease or Multiple Sclerosis. 

Smokers attempting to quit

Providing there are no CHD risk factors or symptoms of impairing respiratory function

Back pain

Chronic

Fibromyalgia

Documented history given

Asthmatics

Controlled and stable. Must carry inhaler when exercising
